Residential Services
MHDS, Inc. provides Residential supports to individuals who choose to live in their own home. MHDS, Inc. provides these services through Supported Living, Semi-Independent Living, Residential Habilitation, Medical Supported Living, Medical Residential Habilitation and Personal Assistance
Supported Living services provide staff support to up to 3 Service Recipients in one home. Individuals receiving this type of service rent or own the home in which they live. The Service Recipient(s) are responsible for all costs associated with their home, with the exception of the staff cost. Service Recipients divide the expenses including the rent, groceries, utilities, and general upkeep of the home. Each Service Recipient receiving any Residential Service will have an Individual Support Plan developed that addresses the person’s goals and needed supports. Staff support is provided up to 24 hours per day as indicated by the Service Recipients Individual Support Plan. Supported Living staff is responsible for helping teach the individual daily living skills that will allow them to increase or maintain their level of ability and independence in the home. Staff may assist with meal preparation, daily hygiene needs, medication administration, and transportation.
Residential Habilitation offers the same staffing supports of Supported Living. Residential Habilitation differs from Supported Living in that the home that the Service Recipients live in is owned by MHDS, Inc. and can accommodate up to 8 Service Recipients per home. In Residential Habilitation, the Service Recipient pays a flat Room and Board Fee that covers the costs of their meals and general household needs.
Medical Supported Living is a Supported Living model that provides specialized staffing supports to Service Recipients who require nursing care by a Registered Nurse (RN) or Licensed Practical Nurse (LPN) a minimum of 4 hours per day. Medical Supported Living is provided to up to 3 individuals who rent or own their home. Medical Supported Living must be ordered by a Physician.
Medical Residential offers the same staffing supports as Medical Supported Living. Medical Residential Habilitation differs in that the home that the Service Recipients live in is owned by MHDS, Inc. The Service Recipient(s) pay a flat Room and Board Fee that covers the cost of their meals and general household needs. Medical Residential must be ordered by a Physician.
Personal Assistance provides up to 24 hour support to individuals who choose to remain in their natural family home. Support Staff assist with daily living tasks such as bathing, cleaning, feeding, and also provides a respite opportunity for the primary caretaker.
Representative Payee Services is also offered to all Service Recipients receiving Residential Services through MHDS, Inc. If the Service Recipient and/or their family members choose for MHDS, Inc. to become the Representative Payee for the individual, the agency is responsible for the payment of monthly bills and oversight of the individual’s checking account and finances.
